A plan can have different Part D premium and cost-sharing details. The $41.33 figure is the national base beneficiary premium used in the statutory premium calculation, not a universal monthly plan premium.
Medicare Part B
The 2027 standard Part B premium is not final yet. The 2026 Medicare Trustees Report currently estimates a standard monthly premium of $209.50 for 2027, compared with $202.90 in 2026.
Projected increase: If the Trustees estimate becomes the final standard premium, it would be $6.60 more per month than the 2026 standard premium.
The official 2027 Part B deductible has not yet been announced. The 2026 annual deductible is $283. We are intentionally leaving the 2027 figure blank until CMS publishes the final amount rather than presenting a forecast as a settled Medicare cost.
CMS explains that Part B premiums and deductibles are determined each year under the Social Security Act. The final 2027 announcement will replace the estimates on this page when it is released.
Medicare Part A
The official 2027 Part A inpatient hospital deductible has not yet been published. For 2026, the Part A inpatient deductible is $1,736 per benefit period.
Part A also has cost sharing for longer hospital stays and skilled nursing facility care. Those 2027 amounts will be updated here when CMS releases the final Parts A & B cost announcement.
Important: The Part A inpatient deductible is charged by benefit period, not simply once per calendar year. That makes it different from the annual Part B deductible.
Medicare Part D
CMS has finalized the 2027 defined-standard Part D deductible at $700 and the annual out-of-pocket threshold at $2,400. CMS also set the 2027 national base beneficiary premium at $41.33.

No. CMS describes the base beneficiary premium as the starting point for calculating a plan-specific basic premium. Actual Part D premiums vary by plan and market. CMS said it expects the finalized 2027 Medicare Advantage and Part D landscape and final average premiums in September 2026.
Under the redesigned Part D benefit, covered prescription drug spending reaches the catastrophic phase after the annual out-of-pocket threshold is met. Beneficiaries continue to have no cost sharing for covered Part D drugs in the catastrophic phase.
CMS announced that the voluntary standalone Part D Premium Stabilization Demonstration will end after 2026. CMS said Part D sponsors now have sufficient experience with the redesigned benefit to support their 2027 bids under traditional market conditions.
Income-related premiums
The 2027 IRMAA brackets have not yet been finalized. Medicare generally uses modified adjusted gross income (MAGI) from the tax return from two years earlier, so a 2027 IRMAA determination will generally look to 2025 tax information.
Your modified adjusted gross income is reported on your federal tax return.
Social Security generally uses that two-year-old tax information when determining whether IRMAA applies.
If your income exceeds the applicable thresholds, an income-related adjustment can be added to your Medicare premium costs.
Don't rely on projected 2027 bracket charts as final. We will publish the actual thresholds and premium tiers here when they are officially released.
